BIO-SECURITY DECLARATION & HEALTH RECORDS SUBMISSION

We look forward to hosting you in the desert! As part of our biosecurity protocols, you will need to submit the following for each horse:

1. A declaration of health and vaccination status
2. Vaccination documents
3. Your Certificate of Veterinary Inspection (CVI) – for out-of-state and international horses only.

All of these should be uploaded utilizing the form below.
